The role of Lead LSC Discharge Planner requires a bachelor’s degree in the field of social or human services from a recognized university. (BHRS certification preferred), Minimum of one-year work experience with children in the mental health field or related social service field. Provide supervision and coordination for operations in the event the CALM Center Program Director is out of the office. Supervise direct reports and assist in the completion of Life-skill coaches performance evaluations in a timely manner. Facilitate discharge planning and family meetings with all clients, provide support and directions in targeted support services such as behavior management, daily living skills, social skills, linkage with community supports, referral to local needed services, advocacy, service coordination, and follow-up on behalf of client and their family who are experiencing emotional and/or behavioral challenges that are significantly interfering with their success at home, school, work or in the community.
