The role of Care Coordinator requires a Bachelor’s degree in the field of social or human sciences from a recognized university. Case Manager II certification or eligible to obtain Case Manager II certification. Minimum of one-year work experience with children in the mental health field or related social service field. Requires the willingness and ability to work irregular hours as needed to ensure quality standards are met in service delivery.
