Intake & Engagement Care Coordinator 23-009 The role of Care Coordinator requires a bachelor’s degree in the field of social or human services from a recognized university. Must have or be eligible for Case Management II certification. Minimum of one year experience in a mental health field or related social service field. Must demonstrate strong communication and organizational skills. Initiate services through a non-diagnostic interview, completing a strengths/needs assessment, and developing a service plan with the client and his/her family to assess service needs. Strong attention to detail and must be able to keep up with multiple priorities. Must demonstrate computer proficiency. Candidate must have a valid driver’s license and reliable transportation.
